Cleaned up issues with PAGE_UNMAPPED and friends

Removed get_free_va_range(), which is now done with VMRs, cleaned up
cases of PAGED_OUT pages (mostly made it panic since we don't swap
properly), and took care of a couple other details related to the
meaning of UNMAPPED, PAGED_OUT, and PRESENT.
